リハビリテーションによる脳血管障害者のスピリチュアリティの評価モデルの検討
―１事例の分析からの一考察―
抄録
本報告の目的は、一事例のスピリチュアリティ評価結果から、リハビリテーションによる脳血管障害者のスピリチュアリティ変容の評価モデルを検討することである。
今回、脳梗塞後左片麻痺の事例のスピリチュアリティを日本語版Stroke Impact Scale3.0（以下、SIS3.0）を用い、面談した。またリハビリテーション中のスピリチュアリティを担当セラピストによる言語的・非言語的観察記録から評価した。
この結果、初回のSIS3.0得点は全項目で低下が認められ、最終回では多くの項目が改善した一方で、Hand functionは低下し、Recoveryについては変化を認めなかった。また担当作業療法士は、初回時、最終回ともに、事例の言動からスピリチュアリティの低下が予測された。
以上から対象者のスピリチュアリティ変容の評価にはSIS3.0などの自己評価と、セラピストによる観察などによる客観的評価が一つの手がかりと推測された。今後さらに多面的な方法の検討が必要と考えられた。
Examination of the evaluation model of spirituality of patients with cerebrovascular disorders through rehabilitation
― A study from the analysis of one case ―
Abstract
The purpose of this report is to examine an evaluation model for changes in the spirituality of patients with cerebrovascular disease through rehabilitation, based on the results of spirituality evaluation of one case.
We interviewed patients with left hemiplegia after cerebral infarction about their spirituality using the Japanese version of Stroke Impact Scale 3.0; SIS 3.0. In addition, spirituality during rehabilitation was evaluated from verbal and nonverbal observation records by the therapist in charge.
As a result, the initial SIS3.0 score decreased in all items, and while many items improved in the final evaluation, hand function decreased and no change was observed in Recovery. In addition, the occupational therapist in charge predicted a decline in spirituality based on the behavior of the cases at both the first and final sessions.
From the above, it is assumed that self-evaluation such as SIS3.0 and objective evaluation such as observation by therapists are one way to evaluate the change in spirituality of the subject. It was considered necessary to consider more multifaceted methods in the future.
