介護老人保健施設での包括的褥瘡ケアシステム導入が経済面へ及ぼす影響
―褥瘡ケアにかかるコスト―

要旨
【目的】介護老人保健施設での包括的褥瘡ケアシステム導入が、経済面へ及ぼす影響を検討することである。【方法】平成23年９月から３年間の入所者759名を対象とし、システム導入後の褥瘡ケアに関する経済面への影響を後方視的に調査した。【結果】総額、クッション購入費、褥瘡委員会人件費には変化はみられなかった。物品代、栄養補助食品代、受診代、受診付添代は減少した。一方、治療およびカンファレンスにかかる人件費、写真代、エアマットレンタル代は増加した。【考察】総額には差はなかったが個々を比較すると、増加がみられたエアマットレンタル代、写真代、褥瘡カンファレンス人件費などは、褥瘡ケアにおける必要経費であり、システム導入により経済面には好影響であったと考えられた。

Abstract
Objective: The objective was to examine the economic aspect of the introduction of a comprehensive bedsore care system in an intensive care facility for the elderly.
Method: I investigated the economic aspect related to bedsore care after the system’s introduction, for 759 people who entered the facility during the three-year period from September, 2011.
Results: No changes were found in the total sum, cushion purchase costs, or bedsore committee personnel expenses. There was a decrease in article charges, supplement charges, consultation charges, and consultation attendance charges. In contrast, there was an increase in the personnel expenses of bedsore conferences, photograph costs, and air mattress rental charges.
Discussion: There was no difference in the total sum, but on comparing the individual items, those items where there was an increase, such as air mattress rental charges, photograph costs, and personnel expenses in bedsore conferences, are necessary expenses of bedsore care, and it was thought that the introduction of the system had a positive economic effect.
